General Data Protection Regulation (GDPR) 

From 25th May 2018, the Data Protection Act (1998) was replaced by the General Data Protection Regulation legislation.  As part of this change we are required to display our privacy notice, which explains how we process the data we hold on you.

We may need to process your data in different areas such as during an emergency, for safeguarding purposes or for public health.  If you would like to see any of our privacy notices, please contact reception who will be able to assist you. 

If you have any questions or concerns about how GDPR might affect you, please contact the surgery and the reception team will direct your query to the most appropriate person. 

Access to Records 

You have a right to access the records we hold for you.  You can view your electronic record via the NHS app, or you can request a copy of your notes via reception.  This can be done verbally or in writing. 

No information will be released to a relative or third party without patient consent unless we are legally required to do so.

Freedom of Information 

Under the Freedom of Information act, if you would like your medical records to be witheld from the National NHS IT Spine, please inform us in writing.  

Your Medical Records and Confidentiality 

The Practice will ensure that patient condfidentiality is upheld at all times by our team. On occasion, your information may be discussed amongst the clinical team to discuss the best course of action for you. 

The Practice records all telephone calls for patient care, security and complaint resolution.  All call data complies with the Practice's Data Protection Registration.
